Neither Do We.
Children with complex medical needs deserve more than a placement—they deserve connection, comfort and continuous care. Upbring is one of the few organizations in Texas licensed to place children with Primary Medical Needs (PMN) in loving, medically trained foster homes.

Join us in creating lasting change for children with complex medical needs.

Children with PMN have serious, often life-threatening health conditions that require highly specialized, 24-hour eyes-on care. This can include:
- Feeding tubes or parenteral nutrition
- Tracheostomies or ventilators
- Colostomies or other surgical interventions
- Ongoing sterile care or nursing routines
- Multiple disabilities or developmental delays
Without the right care, these children are at risk. With the right support, they have a chance at stability, comfort and a better future.
Upbring is one of the few organizations in Texas licensed to place children with PMN. Our program provides:
- Foster families who complete extensive, specialized medical training—often including hospital-based education
- In-home nursing support, available 24/7
- Case management and therapies that support each child’s emotional, physical and developmental needs
- Smooth transitions from hospital to home, including coordination of medication and medical equipment

“PMN placements don’t just happen—there’s intense planning and training involved. But nothing beats the smiles on those families’ faces.”
— Phyllis Christensen, RN, former PMN treatment director

Sharon and Mike
In Nacogdoches, Texas, Sharon and her late husband Mike Williams fostered 27 children—18 of them with PMN—over their 20 years of fostering. From managing seizure care and brain injuries to long-distance medical appointments, their home is a place of healing and resilience.
Sharon now cares for four children, supported by a full-time team of nurses. The children’s needs are complex, but she says the reward is immeasurable.
As of 2024, Upbring has served 45 children with PMN—each story a powerful reminder of what’s possible when we lead with compassion.
Children with PMN often face long and complicated journeys—physically, emotionally and medically. They require hands-on support, coordinated services and caregivers who are both prepared and deeply committed.
At Upbring, we believe this care is a shared responsibility. No one person can do it alone—but together, we can ensure these children are never turned away simply because their needs are complex.
By supporting PMN care, we are:
- Giving children a chance to experience stability and belonging
- Empowering caregivers with the training and tools they need
- Building a stronger system of support for those who need it most
This work may not always be easy—but it is always worth it.
